Facility Use Forms
  • Facility Use Application & Hold Harmless Agreement - Fillable You will need to complete the Application, Hold Harmless Agreement and Acknowledgement of COVID-19 Risk Agreement. Please email the completed form to avonrec@avonct.gov. If your organization has insurance, you will need to also provide a Certificate of Insurance naming the Town of Avon as the Additional Insured and it must be in the amount of at least $1,000,000. Other coverages also apply. See Certificate of Insurance Requirement and Sample Certificate of Insurance for coverage details.
